Registration Forms
Registration Forms can be dropped off at the preschool office or uploaded to our *online dropbox below. If you see the confirmation on your screen after you submit to the dropbox, we have received your submission.
We operate on a first come, first serve basis. Registration for the 2025-2026 School Year opens on Monday, February 3, 2025 @ 9am.

*we have temporarily disabled our online dropbox until we open launch registration on February 3. Please inquire with the preschool office if you are interested in enrollment for the remainder of the 2024-2025 school year.
A $75 registration fee is due when you submit your registration form. You can either pay through the online submission form or send a check written out to “Mount Calvary Preschool”. The registration fee is non-refundable.

If your student has allergies please contact the preschool office at preschool@mountcalvary.org. There are additional forms we will need you to fill out so we have everything on file for your child.
