Registration Forms
Registration Forms can be dropped off at the preschool office or uploaded to our *online dropbox below. If you see the confirmation on your screen after you submit to the dropbox, we have received your submission.
We operate on a first come, first serve basis. Registration for the 2026-2027 School Year opens on Monday, February 2, 2026 @ 8am.

A $75 registration fee is due when you submit your registration form. You can either pay through the online submission form or send a check written out to “Mount Calvary Preschool”. The registration fee is non-refundable.

If your student has allergies please contact the preschool office at preschool@mountcalvary.org. There are additional forms we will need you to fill out so we have everything on file for your child.
